| Elder Resource and Referral Specialist |
Screen calls, provide information to the
community on elder resources; process client intakes; expand and
update computer resource files; record keeping. |
BA preferred; 1 year experience in Human
Services accepted; experience with elders, knowledge of resources
and PC skills helpful. Excellent telephone and interpersonal
skills required. |
| Home Care Case Manager |
In-home assessment of elders
in the community; service planning, providing information and
referral |
BA/BS and experience with
elders preferred; Associates Degree and a minimum of one year’s
experience in a Human Service setting required; must have LSW
within two years of hire. |
| Home Care Registered Nurse |
In home assessments, monitoring, evaluation;
developing care plans enabling elders to remain in the community. |
MA Licensed R.N., BSN preferred; 5 years
nursing experience required, long term care experience preferred
with two years of community based nursing. |
