Position: Home Care Registered Nurse
Supervisor: Nurse Manager
Position Responsibility:
The Home Care Registered Nurse is responsible for participating in the interdisciplinary approach to case management of elders living in the community. This includes assessing, monitoring and authorizing personal care services as well as screening for nursing home placement and diverting appropriate elders into community care plans in accordance with the guidelines set by the Executive Office of Elder Affairs Coordination of Care Agreement and other contracts.
Duties:
1) Assess, implement and monitor the appropriateness of Personal Care Homemaker services.
2) Participate in an interdisciplinary team of Registered Nurse, Case Manager and Program Manager; attend appropriate case consultations and conferences.
3) Screen applicants for nursing home placement, Adult Day Health, 2176 Home Waiver Services, CHHA RN, PERS and HHA services, assisted living and/or other community-based programs.
4) Seek appropriate elders for diversion from nursing home to the community.
5) Complete the Long Term Care Assessment Tool on applicants seeking home care services where appropriate.
6) Provide documentation specific to each program as mandated by the Executive Office of Elder Affairs and MA Health.
7) Attend appropriate agency and community meetings.
8) Participate in agency committees, which would enhance the effectiveness and quality assurance of the medical component of assessed needs.
9) Conduct trainings regarding health issues of the elderly.
10) Perform related work as directed by the Nurse Manager.
Qualifications:
Bachelor’s degree in Nursing or BA in Human Services preferred; Massachusetts Registered Nurse; minimum three years clinical experience, one of which must be in community nursing, required. An additional two years of community-based nursing experience or long term care direct experience may be substituted for a degree (total of five years experience).

Position: Protective Service Caseworker
Supervisor: Protective Service Supervisor
Responsibilities:
The Protective Service Caseworker will assess and investigate Protective Service cases. The Caseworker is responsible for investigating Protective Service reports, assessing the level of client risk, providing crisis intervention services, advocacy, and monitoring of the case to resolve crisis issues.
Duties:
1) Investigate Protective Service reports as assigned by the Protective Service Supervisor.
2) Provision of crisis intervention, advocacy and casework services.
3) Completion of appropriate paperwork and maintenance of client files, including computer database, as defined by the Protective Service Supervisor and as regulated by the Executive Office of Elder Affairs.
4) Development and maintenance of ongoing service plans to assist in the resolution of Protective Service cases.
5) Accessing appropriate mental health, medical resources and legal assistance when cases require this attention.
6) Liaison and clinical consultation with case management staff when appropriate.
7) Assist with community education efforts as assigned by Protective Service Supervisor.
8) Utilization of court system when necessary as indicated under the MGL Chapter 19A 651 CMR law and other mandates.
9). Case coverage and liaison efforts with other staff members of the Protective Service team.
10)
Coverage responsibilities for the Protective Service team including participation in the after hours beeper system.
11)
Other duties as assigned by the Protective Service Supervisor.
Qualifications:
MSW preferred BSW or BA in Human Services field required; two years minimum experience with elders, crisis work or case management; must be LSW eligible within two ½ years of hire; effective negotiation and problem solving skills required; excellent writing and communication skills required; basic computer skills helpful.
