Mental Health Care Management
Undiagnosed and under treated mental illness increase risk for medical illness, falls, self-neglect and homelessness. Often, the ability to function independently is hampered by their difficulty in managing anxiety, depression, memory loss, or other symptoms associated with mental health. In addition, mental illness often leads to isolation and difficulty accessing long-term services. To assist this population, Elder Services developed a Mental Health Care Management Program. The staff is highly skilled, and specially trained to deal with many of the challenges related to living with mental illness. They are sensitive to an elder’s rights for dignity, respect, and good care at all times. Their relationship with these elders is built over the long-term to ensure trust and a sense of safety.
