H Adult Housing Complexes

·      Apartment style living for older adults

·     Community Housing Authorities and private companies manage apartment buildings

·      Residents may qualify for rent subsidies based on income or pay fair market value for an apartment

·      Supportive living services may be arranged in many of the complexes

·      Services may include meals, homemaker, personal care

·      Private pay may be arranged

H Assisted Living

·      A design combining housing and supportive services in a private or semi-private apartment

·      Physical layout, number of units, and fee structure vary greatly.

·      Supportive services may include 2-3 meals daily, housekeeping and help with personal care.

·      Optional amenities may include services such as transportation, social or educational activities and health related services.

H Congregate Housing

·      Shared living experience for older or disabled adults located within an apartment complex

·      Residents share common space: kitchen, living room and dining area

·      Maintain their privacy with their own apartment or private bedroom.

·      Congregate Coordinator and supportive services are available

·      Elder tenants (60+) receive home care services which may include   homemaking, laundry and shopping and meal preparation

H Family Care for Adults

·      A supervised housing environment offering older or disabled adults to live with a provider family

·      Ideal for those requiring help with daily living activities

·      A professional nurse and social worker team monitor the placement process  so the placement is successful for both the provider family and the client.

·      Placement is based on common interests, physical layout, location, and compatibility between provider and client

·      Medicaid and private pay accepted

·      Host providers receive monthly compensation to provide room and board.

H Nursing Homes

·        Long term care facilities providing around the clock medical care

·        Licensed by the state

·        Provide skilled nursing and related services to ill, injured or disabled adults

·        Residents have one or more health diagnoses requiring 24/7 medical care

·        There are guidelines for placement

·        Costs may vary from one facility to another
