Eating right extends life and knowing what’s “right” is the key. Elder Services has a team of Registered Dietitians available to help you learn how to eat healthy to be healthy!
Individualized In-Home Nutrition Counseling
Our dietitians will come to you! A typical visit includes review of medical history and discussion of eating habits/foods consumed to get a snapshot of nutritional health. Evidence-based advice is provided, and obtainable nutrition goals are set. Initial appointments take 60 minutes. Follow-up appointments are available as necessary, lasting 30-45 minutes. Common conditions addressed include (but not limited to) weight loss, weight gain, diabetes, heart disease, allergies and GI disorders. The cost for this service is dependent on the consumer’s current care enrollment with Elder Services.
Medical Nutrition Therapy
Individualized in-home nutrition counseling for persons with Medicare who are diagnosed with type 2 diabetes and/or chronic kidney disease (CKD). This service is not limited to Elder Services clients. The Registered Dietitian delivers up to three nutrition counseling sessions per year. Evidence-based advice is provided, and obtainable nutrition goals are set. Initial appointments take 60 minutes with follow-ups lasting 30-45 minutes. Group MNT nutrition sessions also available. The cost for this service is covered by Medicare.
Interactive nutrition lessons are provided at community organization within the Merrimack Valley service area. These events are open to the public and free for all attendees. Check our calendar and Facebook page for events in your area.
