We have a team of Registered Dietitians available to help you learn how to eat healthy to be healthy!
Individualized In-Home Nutrition Counseling
Our dietitians will come to you. A typical visit is about an hour long and includes a review of your medical history and a discussion around eating habits. The dietitian provides advice and sets nutrition goals. Follow-up appointments are available. Common conditions addressed include weight loss, weight gain, diabetes, heart disease, allergies and GI disorders. The cost for this service is dependent on the consumer’s current care enrollment with Elder Services.
Medical Nutrition Therapy
In-home nutrition counseling is available for persons with Medicare who are diagnosed with Type 2 diabetes and/or chronic kidney disease. This service is not limited to Elder Services clients. A Registered Dietitian delivers up to three nutrition counseling sessions per year. Group nutrition sessions also available. The cost for this service is covered by Medicare.
Interactive nutrition lessons are provided through community organization within our service area. These events are open to the public and free for all attendees.
